Service Description: Resources to assist adult stroke survivors and their caregivers to find community agencies that provide direct service or the link to other programs that support reintegration into the community

Trillium Health Partners - Queensway Health Centre is the designated Regional Stroke Centre for the West GTA Stroke Network * provides stroke care throughout the whole continuum from the Emergency to Ambulatory Rehabilitation * consists Comprehensive Stroke Unit (20 beds) where patients transition from acute to rehabilitation in a seamless fashion by staying in one location and keeping the same stroke professional care team

Stroke clinic services include:
  • comprehensive stroke care management and education
  • reduce wait times to be seen by stroke neurologists
  • rapid access to diagnostic tests
  • early identification of risk factor
  • individualized treatment plan
  • data collection and data analysis to support continual  practice improvements
  • clinical outcomes and support research

The West GTA Regional  Secondary Stroke Prevention Clinic is housed at Trillium Health Partners - Mississauga Hospital
