Service Description: Nursing and rehabilitation services designed to support seniors and vulnerable individuals

Nursing Services include:
  • wound care
  • ostomy management and teaching
  • continence nursing
  • palliative care
  • iv therapy
  • chronic disease management
  • dialysis
  • care of medically fragile children
  • medication management

Rehabilitation Services:
  • dietetics and nutrition
  • social work
  • physiotherapy
  • occupational therapy
  • speech language pathology

Personal Support
In-home assistance with activities of daily living including:
  • bathing, dressing, toileting, meal preparation, light housekeeping, exercising and medications, as well as other routine activities of daily living

Other services:
  • relief and support for caregivers
  • respite care
  • overnight care


Services provided in homes, workplaces, and schools

Fees: CarePartners provides most of its services to eligible patients through its funder, Ontario Health atHome. CarePartners also provides family-funded services.
Application: By appointment only; call the local CarePartners location for more information.
Eligibility / Target Population: Persons requiring healthcare services in urban, rural and remote communities across Ontario.
