Nursing Services include:- wound care

- continence nursing

- chronic disease management

- care of medically fragile children
- medication management
Rehabilitation Services:
- dietetics and nutrition
- social work
- physiotherapy
- occupational therapy
- speech language pathology
Personal Support
In-home assistance with activities of daily living including:
- bathing, dressing, toileting, meal preparation, light housekeeping, exercising and medications, as well as other routine activities of daily living
Other services:- relief and support for caregivers
- respite care
- overnight care
Services provided in homes, workplaces, and schools

CarePartners provides most of its services to eligible patients through its funder, Ontario Health atHome. CarePartners also provides family-funded services.
