Service Description: The IADTP helps reduce non-emergency Emergency Department visits and/or hospitalization, allowing for faster discharge when visits are necessary, and reduces recidivism, as the hospital may refer patients directly to the program. It allows for seamless navigation, access - and transition to the appropriate level of care

Application: Self referral or professional agency referral to be faxed
