Service Description: Social Work supports and services include:
  • One-to-One Supportive Counselling

Topics may include:


  • living with acute and chronic health challenges; coping with illness-related anxiety and depression; adjusting during times of transition; dealing with anxiety related to testing and procedures, and with fear of dying; facilitating legacy work projects
  • managing caregiver responsibilities and avoiding burnout; dealing with anticipatory grief
  • coping with bereavement, loss and grief

Additional Social Work services and supports include:

  • Support Groups
  • Assistance navigating the health care system
  • Advanced Care Planning
  • Patient Advocacy
  • Guidance on funding sources and assistance with the application process
  • Linkages and referrals to complementary supports
  • Education and Resources

Fees: Free

Eligibility / Target Population: Individuals living with life-limiting illness and their loved ones *individuals who have experienced the death of a loved one
