Care for adults with complex diabetes
Offers
- access to a team of diabetes experts. This includes an Endocrinologist, Nurse Practitioner, Certified Diabetes Educator are both nurses and dietitians, Social Worker and Pharmacist
- personalizes care to meet client's needs
- partners with programs and clinics across Osler to ensure client receives coordinated care

Referrals can be made by any family doctor or medical specialist in the communities served by Osler

Eligibility / Target Population:
|
Adults 18 years of age or older, have comorbid conditions associated with their diabetes, such as vascular disease, renal failure, neuropathy, impaired vision, mental illness, financial difficulty accessing care, have inadequate glycemic control and/or significant co-morbidities which impact on glycemic control; and/or, have had recurrent diabetes emergencies or have been hospitalized with poorly controlled diabetes and higher readmission rate
