Service Description:

Ontario Ministry of Community and Social Services agency, helps adults with developmental disabilities connect to services and supports in their communities.
Services include:- residential support
- caregiver respite
- community participation supports
- professional and specialized services
- person-directed planning
- other support to help people with developmental disabilities become more involved in their communities
There are nine DSO offices across Ontario

Assessor from DSO will help person with disabilities to complete application

Eligibility / Target Population:
|
Ages: 16 years and up
Individual must have a documented developmental disability, and meet age and residency requirements.
