Service Description: Retinal Eye Screening Program: offers easy access to medical professionals for those with diabetes. Referring physicians send patients to a local OTN site where digital images of the eye are taken and sent to an ophthalmologist for assessment. A report with diagnosis and treatment recommendations is then sent back to the physician

Application: Referrals: physicians should submit an Opthalmology retinal screening request
Eligibility / Target Population: People at risk of diabetes, pre-diabetes, type 1 diabetes, type 2 diabetes and gestational diabetes
