Service Description: Inpatient consult service is available to patients admitted to hospital and their families

Offer:
  • recommendations to optimize pain and other symptoms
  • information around what to expect, discussions regarding options, and establishing goals of care
  • transfer to the acute palliative care unit for management of complex symptoms or end of life care (when indicated)
  • support with planning for palliative care at home or other facilities
  • follow up in outpatient clinic
Location: 9th floor, turn right when exiting the elevators, turn right again

Fees: Most services covered by OHIP
Application: Physician or Nurse Practitioner Referral required
Eligibility / Target Population: Available to patients admitted to hospital and their families

Supportive Palliative Care Patients referred for palliative care are those suffering from incurable and/or life-limiting illnesses of a cancerous and/or non-cancerous nature. Palliative Care is appropriate for patients in all disease stages, from those undergoing active medical treatment to those who are nearing the end of life. At all stages, the goal is to provide pain and symptom management thereby improving quality of life
