Service Description: Service coordination and support to any family or person where the person has been identified as having a diagnosis of a developmental/intellectual disability and/or autism

Service Coordination Program Children – Under 18 Years of Age (Dufferin County)
A family-centered service that plans and coordinates community programs, resources, funding opportunities and supports based on the identified needs of the individual. Provides service coordination and support to any family or person where the person has been identified as having a diagnosis of an intellectual/developmental disability (specifically means a sub-average general intellectual functioning with an IQ at or below 70 (2nd percentile or lower) and/or Autism Spectrum Disorder

Service Coordination Program for Adults 18 Years of Age and Over (Dufferin County)
Service Coordination for adults with a documented developmental/intellectual disability in Dufferin County as referred by Developmental Services Ontario (DSO). Service Coordinators assist with future planning, help navigate through the adult service system and are responsive with active participation by the adult. Service Coordinators provide linkages to services and offer a solution focused service and support with the adult/family. Services promote social inclusion, individual choice, and independence. To be eligible for Ministry funded adult services, individuals must apply to Developmental Services Ontario (DSO)
