Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health including:
  • Dental care
  • Diabetes education and support
  • Healthcare services
  • Healthcare promotion programs
  • Sexual health and harm reduction services

The WellFort group, provides primary health care focusing on health promotion and prevention to the residents of Bramalea/Brampton and Malton

The Group comprises of:
  • Bramalea Community Health Centre
  • Four Corners Health Centre

Fees: Most services covered by OHIP
Application: Complete a client assessment form in person * call to make an appointment
Eligibility / Target Population: Live in the area served * be a member of one or more of the following priority populations: Youth (ages 13-25) * adults 65 years and older * newcomers to Canada (less than 5 years) * racialized communities * people facing mental health conditions or substance use issues * people who are homeless or at-risk of homelessness * families who are under-resourced
Mobile services: 65 years of age and older * having difficulty accessing primary health care due to physical, cognitive or psychiatric difficulties * willing to transfer their primary health care to Bramalea Community Health Centre * not living in a nursing home facility * not requiring palliative care * ability to provide consent to participate and undergo an intake assessment
