Service Description:

Health centre focused on family and community health * health
professionals assess and treat non-life-threatening injuries or
illnesses * referrals to other local health services and personal development groups
Services offered:
Family practice medical services - involve a multidisciplinary healthcare team that includes family physicians, nurse practitioners, and registered nurses. Services include:
- one-on-one medical appointment for individual clients
- well baby visit
- immunization for infants and young children
- adult health maintenance and management of chronic diseases
- pregnancy care for women who are up to 27 weeks pregnant
- counselling for mental health and substance use disorders
- chronic disease management- diabetes education, physiotherapy, chiropody, registered dietitian services
- special health education activities and programs for clients and community members (pre-registration may be required)
- case management and system navigation
- low vision clinic (In collaboration with Vision Loss Rehab Canada)
Other Services
Health Promotion Program - family and community support, community capacity building - client support
Seniors Services - programs via teleconference, programs via zoom, family caregiver programs, in-person programs (Somali Seniors program), digital support services
