Service Description: Interprofessional Primary Care team focused on family and community health * multidisciplinary team of registered nurses, social workers, registered dietitians, nurse practitioners, pharmacists, and health educators, provides certain programs and services * team focuses on family and community health and supports patients in their coordination of care
  • Healthy Families Program - One-on-one and group sessions
    • Staying Healthy during Pregnancy - Prenatal program focusing on issues related to prenatal, early postpartum, and infant care - aim to attend in the first or second trimester of pregnancy
    • Introduction to Breastfeeding - Information on the basics of breastfeeding - aim to attend while pregnant
    • Introducing Solid Foods to your Baby - Information for parents who are planning on transitioning their infant to solid foods - aim to attend before infant is 6 months old
    • Raising a Happy, Healthy Eater - Information on how to manage and prevent picky eating habits in toddlers and young children

  • Lung Health and Smoking Cessation Program: STOP for FHT
