Service Description:

For patients living with COPD or chronic heart failure, Telehomecare can help
Through a telephone and the internet, patients will be linked with a registered nurse at Brampton Civic. The nurse will:
- remotely monitor patient's health at home
- contact patient to provide support and advice if patient's condition changes
This program is funded by the Ministry of Health and provided in partnership with the Ontario Telemedicine Network

Eligibility / Target Population:
|
Patients with an established diagnosis of a congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD) Repeated emergency department visits and/or hospital admissions Capable of using in-home monitoring equipment
