Service Description:	Inpatient Service: Geriatric Emergency Management (GEM) Nurses - Osler Emergency Department includes GEM nurses. These nurses: assess patients to identify their health, functional, emotional cognitive and social needs link seniors to community services provide education to patients, their families and Emergency Department staff support seamless transitions between the Emergency Department and community health care services
