Service Description:

Help for patients to understand:
- how COPD affects their lungs
- medications and why, when and how to take them
- what causes a COPD “flare up” and how to avoid one
- when to seek medical attention
If patient is admitted to hospital, referral may also be made to an Urgent COPD Clinic upon discharge. This helps the patient get follow-up therapy, if needed
