Service Description: Assessment of older adults in crisis who have: dementia or cognitive decline, mental health, substance use disorder and/or other neurological conditions, and experiencing responsive behaviours. This can include persons unable to cope and manage current situations that involve the cognitive, social, environmental, physical, and social life.

Conduct risk and safety assessments, provide supports and safety plans, and referrals to community resources.

Referrals can be made by calling the Crisis 24.7 number, or by completing the Crisis 24.7 "Alert Form": https://cmhapeeldufferin.ca/wp-content/uploads/2020/06/New-Crisis-Alert-Form-Jun2020.pdf, and faxing it to the fax number on the "Alert Form"

Eligibility / Target Population: Seniors (65 years of age+), or persons over 50 years of age with signs of early cognitive decline/living in the Dufferin or Caledon area
