Hours: Diabetes Care Program: Mon-Fri 8:30am-noon * 1pm-4:30pm * closed for lunch noon-1 pm
Service Description: Basic to advanced level diabetes education and ongoing care to children and families, youth and adults who are at risk or have type 1 diabetes, pre-diabetes, type 2 diabetes, pre-gestational diabetes, and insulin pump starts and management * program consists of a multidisciplinary team including registered nurses (CDE), registered dietitians (CDE), social workers and physicians

Services are available for:
  • pre-diabetes
  • type 2 diabetes
  • type 1 diabetes
  • paediatrics (children)
  • diabetes in pregnancy and gestational diabetes
  • insulin pump therapy

Location: On the lower level of HHCC – down the same hallway as the cafeteria



Fees: Most services covered by OHIP
Application: Appointment available on a self-referral with the exception of specialty clinics * endocrinology and pediatrics require a physician referral
Eligibility / Target Population: Children and families, youth and adults who are at risk or have type 1 diabetes, pre-diabetes, type 2 diabetes, pre-gestational diabetes, and insulin pump starts
Languages: English ; Korean
Area Served: Dufferin County, Grey County, Peel Region, Simcoe County, and Wellington County
