Service Description:
|
Basic to advanced level diabetes education and ongoing care to children and families, youth and adults who are at risk or have type 1 diabetes, pre-diabetes, type 2 diabetes, pre-gestational diabetes, and insulin pump starts and management * program consists of a multidisciplinary team including registered nurses (CDE), registered dietitians (CDE), social workers and physicians
Services are available for:
- pre-diabetes
- type 2 diabetes
- type 1 diabetes
- paediatrics (children)
- diabetes in pregnancy and gestational diabetes
- insulin pump therapy
Location: On the lower level of HHCC – down the same hallway as the cafeteria
