Service Description:

Counselling and support for people with type 1 diabetes and type 2 diabetes (on insulin or insulin pump), gestational diabetes * help to patients and families to understand diabetes and how to self manage the disease - test blood sugars, take insulin, manage medications, make healthy food choices
A diabetes nurse and dietitian will assess patient's health and provide education tailored to your needs. Other services include:- appointments conducted in person
- insulin pump program
- insulin adjustment classes
- gestational diabetes classes
- paediatric diabetes program
- transitional program for young adults moving from a paediatric program
- referral to a social worker
- in-services provided to schools and daycares (in-person or virtually)
- in-services provided to health care providers (hospital or community)
