Service Description:
|
Provides assistance and support to clients dealing with problems associated with mental illness
Program includes
- individual and supportive sessions to clients and their families
- psycho-education for mental health related issues
- assist in stabilizing mental health symptoms
- individual assessments and care plans offered
- home visits (based on client needs)
- community outreach, collaborate with community partners
- access to group programs that are linguistically and culturally appropriate
- community outreach
Focuses on
- educating community about mental health and related issues
- help client understand the illness, symptoms and importance of medication in the recovery process
- encourage family members to become active participants in client`s recovery
