Service Description:

Support for clients with the initial needs assessment and connection with appropriate resources * at the end of the visit clients will have a support plan in place
Steps involve:
- initial assessment
- service planning
- service partner referrals
- support plan in place
Resource connection:
- wellness and support groups
- counselling and psychotherapy services and community referrals
- support for people who have experienced abuse

Eligibility / Target Population:
|
Services for people living or working in Peel and Dufferin area. For children under 12, the presence of a parent or a guardian is required at the first meeting, permission of both parents required for children
