Service Description: Provides assistance and support to clients dealing with problems associated with mental illness 

Program includes
  • individual and supportive sessions to clients and their families
  • psycho-education for mental health related issues
  • assist in stabilizing mental health symptoms
  • individual assessments and care plans offered
  • home visits (based on client needs)
  • community outreach, collaborate with community partners
  • access to group programs that are linguistically and culturally appropriate
  • community outreach


Focuses on
  • educating community about mental health and related issues
  • help client understand the illness, symptoms and importance of medication in the recovery process
  • encourage family members to become active participants in client`s recovery

  • Long Term Case Management
  • Early Psychosis Intervention Program
  • Concurrent Disorder Case Management
  • Mobile Crisis Rapid Response Team (MCRRT)
  • Concurrent Disorder Wellness Support Group (Brampton office)
  • Mobile Community Support Group (MCSG)
  • The Ontario Structured Psychotherapy (OSP) Program
