Service Description: Comprehensive diabetes education for people living with type 2 diabetes and those at risk of developing diabetes (pre-diabetes)

The program and services are provided by certified diabetes nurse educators and certified diabetes registered dietitians * group or private appointments are available

Educating people with type 2 diabetes to:

  • reduce risk of complications
  • deal with emotions connected to the disease
  • manage blood sugar levels
  • cope with stress
  • control weight
  • prevent and manage foot problems
  • take medications correctly
  • develop a healthy eating plan
  • meet and learn from other people with diabetes


Educates people at risk of developing diabetes to:

  • reduce blood sugar levels
  • lose weight
  • label read /make healthy food choices
  • create a realistic physical activity plan
  • take medications correctly


Contact the Diabetes Education Program for information. In person appointments are available if required. Currently individual appointments are in person, by phone or Zoom

Diabetes Community Outreach: diabetes health education provided at community locations (religious, community organization, school, or work place), for groups of five or more adults living with Type 2 Diabetes or Pre-Diabetes

Fees: Most services covered by OHIP
Application: Complete a client assessment form in person or via phone * call to make an appointment. Self referral or Doctor referral
Eligibility / Target Population: Over the age of 18 years, living with type 2 diabetes, pre-diabetes or at high risk for developing diabetes
Languages: English * Hindi * Marathi * Polish * Punjabi * Urdu
