Service Description:

Coordinated Access and Intake is primarily a telephone-based service that provides a single point of access for children, youth and their families wishing to access a wide range of mental health programs and services
The System Access and Intake Team, a part of the coordinated intake network for Peel, is operated by EveryMind on behalf of and in association with Associated Youth Services of Peel, Trillium Health Partners and William Osler Health System. A critical component of the service is the agreed-upon system case transfer protocol that facilitates continuity of service across the partners. This protocol was created in order to facilitate the process of transferring client from one organization's services to another partner organization, without necessitating another intake
During the intake process, current information pertaining to the client's presenting problem(s) is gathered using a standardized and systematic format. Use of the interRAI ChYMH Screener+ has been implemented as part of the fully integrated intake interview for children 4 and up. Based on information collected and in consultation with the client, a decision is made regarding the service which will most appropriately meet the identified needs of the child/youth
The Access and Intake Specialist facilitates the client's access to services by providing the information already gathered through the Intake process to the identified child and youth mental health service provider.
